To start the Ritual of Anguish, enter the Hanged Man’s Hall with at least two Sandscorched Shackles and two Pincushioned Dolls. Once inside, you'll need to eliminate the monsters that inhabit the hallways before proceeding to the main arena containing the altar. When you interact with the Altar, the menu for the Ritual of Anguish will pop up.

If you're ready to commence the ritual, place the shackles and dolls into the bowl at the altar, which will permanently consume them.
